*英文姓名欄位僅接受英文字母與空白字元，請勿輸入其他符號。
* 護照號碼與身分證號碼欄位請輸入英文字母(A-Z)或數字(0-9)，身分證號碼最多可輸入八字元，若您輸入身分證號碼長度為八字元，前二字元必須是英文字母；若您的護照號碼或身分證號碼有括號或斜線，請勿輸入括號內或斜線前後數字。
*請依護照上出生地據實勾選。
* For English name: only English alphabets and space will be accepted. Do not key in any other symbols.
* For Passport No. and ID No.: English alphabets or numerical digits only. If your ID No. contains 8 digits/alphabets, the first two characters must be English alphabets. In case your Passport No. includes a parenthesis or slash, please do not key in the digits/alphabets in the parenthesis or before/after the slash.
*Choose your place of birth as appears in your passport .
