🔺The winner will be contacted via the email address provided during entry. Please download the “winner’s receipt” form and fill it out.
🔺Please print the form and mail to the designated address. The mail must be postmarked by September 8, 2023.
🔺Please make sure that the name, telephone and address on the winner’s receipt are correct in order to deliver the prize.
🔺If the winner fails to send back the winner’s receipt before the due date, information is incorrect, or cannot be contacted, he/she will be deemed a waiver of the right to claim the prize.
